What Mounjaro is

Mounjaro® (tirzepatide) injection 0.5 mL is a once-weekly, injectable prescription medicine used together with diet and exercise to help improve blood sugar (glucose) in people with type 2 diabetes. It is indicated for adults and children 10 years of age and older. It is not known if it is safe and effective in children under 10.

Mounjaro is not insulin. It works by activating both GIP and GLP-1 pathways, which helps the body regulate blood sugar. Individual responses vary, and treatment is typically adjusted based on clinical needs and guidance from a healthcare professional.

Dosing options and what to expect

Mounjaro is available as a pre-filled single-dose pen in multiple strengths: 2.5 mg and 5 mg per 0.5 mL. Treatment commonly begins at 2.5 mg once weekly for 4 weeks, then increases to 5 mg once weekly for at least 4 weeks, with additional dose options available if more glucose control is needed. The maximum dose in adults is 15 mg once weekly, and the maximum in children 10+ is 10 mg once weekly, as determined by a prescriber.

Some people experience side effects—most commonly nausea, diarrhea, decreased appetite, vomiting, constipation, indigestion, and stomach pain. In clinical experience described in the brochure, stomach-related effects such as nausea, vomiting, and diarrhea are more likely during dose increases and may lessen over time. If side effects are bothersome or persistent, it’s important to speak with a healthcare provider.

How it’s used, storage, and key safety notes

How it’s used (once weekly): Choose a consistent day each week. If you need to change the day, ensure at least 72 hours between doses. If a dose is missed, it may be taken within 96 hours (4 days); otherwise skip it and take the next dose on the scheduled day. Do not take two doses within 3 days of each other. Mounjaro is injected under the skin (subcutaneous) in the abdomen or thigh, or by another person in the back of the upper arm. Do not inject into a vein or muscle. If you also use insulin, do not mix them in the same injection.

Pen basics: The single-dose pen is used by placing it on the skin, unlocking it, and pressing/holding the injection button until the injection is complete (the pen provides clicks to indicate start and completion). Used pens should be disposed of in a sharps container (or a suitable puncture-resistant container per local rules).

Storage and handling: Store pens in the original carton to protect from light. Keep refrigerated at 36°F to 46°F (2°C to 8°C) and do not freeze. If needed for travel, it may be kept up to 21 days outside the refrigerator as long as the temperature stays below 86°F (30°C). Because the pen contains glass parts, handle carefully and discard if dropped on a hard surface.

Important safety warnings (summary): Mounjaro carries a warning about possible thyroid tumors, including thyroid cancer; do not use it if you or your family have had medullary thyroid carcinoma (MTC) or if you have MEN 2. Serious risks can include pancreatitis, low blood sugar (especially with insulin or sulfonylureas), serious allergic reactions, dehydration leading to kidney problems, severe stomach problems, vision changes, and gallbladder problems. Tell your healthcare provider if you notice symptoms such as a neck lump/hoarseness/trouble swallowing, severe abdominal pain, signs of low blood sugar, allergic symptoms, or persistent vomiting/diarrhea. If you take oral birth control, discuss it with your provider, as effectiveness may be reduced for a period after starting or increasing the dose.
